Complete the following to schedule your appointment online:

Appointments
Select a location
Click on the Appointments menu option to show a list of locations. Select Location
Select Preferences
Select from the Date, Times, and Provider Preferences
Once you have selected a location, you will need to select your preferences of dates, times, and specialties of the providers you would like to see. Then click on Find Appointments to see a list of available times and dates.
Schedule
Schedule an initial Therapy Appointment
Select the time from those available, enter your personal information to be passed to the provider, and click Schedule Appointment. You will then be contacted to verify your personal information within the next 3 Business Hours (Monday - Friday 9 AM to 5 PM) at the phone number you provide. If your appointment is within 3 Business Hours, we will verify your information upon arrival at your appointment.
